Organizations interested in applying for ADGs will be asked to complete the Electronic Grant Application on the HCF website. Organizations that lack the capacity to complete electronically may submit the original and two hard copies of their proposal by mail.

Proposal Package

The following information should be included in your Proposal Package:

  • The Health Care Foundation of Greater Kansas City Grant Proposal Cover Page (form provided on the web site). The cover page will provide information regarding your project and organization.
  • Proposal narrative up to 10 pages which describes the following:
    1. Clearly state the purpose and amount of grant request.
    2. Organization’s mission and history
    3. Problem or need to be addressed by your project and/or program
    4. Issues affecting your proposed target group(s) or community(ies)
    5. The project, through a summary of the timeline, activities, partners, and other factors involved
    6. Client and/or community outcomes and how they will be measured
    7. How project will be sustained both financially and organizationally, at the conclusion of Foundation funding
    8. If joint project, attach letters from each participating organization; specify for each participant their roles, functions and cost
  • Proposal Attachments
    1. Budget worksheet on our web site (click here to open)
    2. Copy of your organization’s most recent IRS 990 Report (nonprofit tax return).
    3. Most recent audit
    4. Board approved current fiscal year operating budget
    5. Roster of board of directors with diversity information
    6. Governmental entities must include: enabling statute/legislation or official description of purpose; authorized budget or current financial statement; list of elected/appointed officials who oversee the entity performance.

Submit the original and two copies of the ADG Proposal and all attachments, excluding the IRS 990 and financial audit report.  Only one copy of each of those attachments is needed.
